Thankyou for selecting Outdoor Adventure Camps for your school camp venue. The following documentation is relevant for all camp participants.
Forms to be completed by participants / parents and returned to the School
- Medical form for participants (1 page)
- Asthma form if applicable (2 pages)
- Parental consent form (1 page)

- Clothing and equipment requirements list for participants (1 page)
- Directions to Outdoor Adventure Camps (1 page)
Additional Information for Camp Co-ordinators
- In the week prior to camp, we will contact you to obtain final numbers and any information regarding dietary issues.
- A tax invoice for the camp costs will be faxed to the school on completion of the camp.
- On previous camps we have taken photographs of students participating in the outdoor program and these photos have been posted on the Gallery page of our website. Please note that we make no reference to either the student’s name or the name of the school. Students get a thrill from seeing photos of themselves and friends involved in a range of camp activities. Please complete the attached Photograph Display Authority if you wish to have your school’s snapshots on our Gallery page.
